Staff members with changes in address or telephone number should also notify the Personnel Office in writing. The 'Change of Personal Data' form is obtainable from the Personnel Office (Tel. 2609 7291), or may be downloaded from its website ( http://www.cuhk.edu.hk/personnel). Change o f personal data may have i mp l i c a t i ons on the appointees' entitlement to benefits at the University. Below are some examples: Fringe Benefits* Examples of Changes in Personal Data Affecting Eligibility for Benefits Children's education allowances • New-boms should be reported so as to include them as eligible members. Housing benefits • Change of marital status/spouse's employment should be reported for clearance of the Rules for the Prevention of Double Housing Benefits. • Change of family size (e.g. marital status/new-borns) should be reported as it may affect the rates of Private Tenancy Allowance. Passages • New-borns should be reported so as to include them as eligible members.
