Applications endorsed by department chairmen and faculty deans, or unit heads concerned should be forwarded to Mrs. Betty Lo, personnel manager, on or before the deadline for submission to the relevant university committees for consideration. Detailed information and relevant application forms are obtainable from faculty/unit offices or the Personnel Office's website at www.cuhk.edu.hk/personnel/res/webtrinf/external. htm. They should arrange to renew their employment visas within four weeks prior to the expiry date. Application for extension of employment visa requires a sponsorship letter from the University to certify the staff member's employment status. Staff members may approach the Personnel Office (2609 7332/ 7272/7292) for the said sponsorship letter. Staff members can make an appointment for visa extension via the Internet ( http://www.esd.gov.hk). This service however is not available to those required to file their applications in the Immigration Department's headquarters. 更改強積金計劃僱員自願供款百分比 MPFS — Change of Employee Voluntary Contribution 強積金計劃成員可於每年四月一日及十月一日，更改僱員自願供款的百分比。 倘欲由二零零四年四月一日起更改該百分比者，可向財務處薪津及公積金組索 取更改表格，或從財務處網頁（ http://www.cuhk.edu.hk/bursary) 下載，填妥後於 二零零四年三月十九日（星期五）或之前送達或傳真（二六零三七八九零）至薪津 及公積金組。 一 查詢或索取表格，請與財務處薪津及公積金組聯絡（內線七二三六或七二五 一）。 Pleased be reminded that members of the Mandatory Provident Fund Scheme (MPFS) can change the nominated percentage of Employee Voluntary Contribution to the MPFS twice a year. The changes are effective on 1st April and 1st October. MPFS members who wish such changes to be effective from 1st April 2004 should complete the 'Arrangement for/Change of Voluntary Contribution for Employee' form, which can be downloaded from the Bursary website at http://www.cuhk.edu.hk/bursary or obtained from the Payroll and Superannuation Unit of the Bursary. The completed form should be sent to the Payroll and Superannuation Unit by mail or fax (2603 7890) on or before 19th March 2004 (Friday). Fee rebate has not been reflected. # 少於 0.005% Less than 0.005% 體檢計劃 Health Screening Programme 大學行政事務委員會批准大學保健處以合理價格為大學教職員及家屬舉辦體檢 計劃。體檢計劃是一項全面綜合性的健康檢查，基本費用為六百元，如需特別檢 驗，須額外收費。現有的入職身體檢查，以及與工作有關的定期檢查或其他健康 檢查的服務及費用不受影響。 保健處已於二月七日推行體檢計劃。為免影響現有服務，體檢計劃只安排於星 期六進行，而年滿四十五歲的教職員暨親屬及退休者將獲優先安排。醫生會講解 及建議一系列合適的健康檢查，其後再約見以解釋化驗結果及提供健康指引等， 並會發出體檢報告。體檢發現的健康問題，可循正常診病安排接受治療。 查詢請致電二六零九六四三六（簡太）；預約則請致電二六零九六四三九。 註：服務包括健康歷史、體格檢查、醫療報告、小便常規檢查、糞便常規檢查、全血計數、腎 功能、血葡萄糖（空腹）、血脂（膽固醇及三酸甘油酯）及肝功能。如需肺部 X 光片或心電圖’ 須各付一百元，而其他特別檢驗則須付成本價。 The Administrative Affairs Committee of the University has recently approved the launch of a Health Screening Programme by the University Health Service (UHS) for eligible 6 No. 235 19th February 2004
